Thoracic aortic aneurysm: a late complication of coarctation repair
نویسندگان
چکیده
A 67-year-old female patient had a patch repair of an aortic coarctation in 1972 and a mechanical aortic valve replacement in 2002 for a bicuspid valve. It was noted at the time of the repair that the aorta was ectatic and regular follow-up was suggested. She was followed up with yearly echocardiograms. It was noted that the diameter of descending aorta had increased to from 4.5 cm in 2013 to 5.4 cm in 2014 and a computed tomography (CT) aortogram revealed an 8-cm saccular aneurysm of the aortic arch around the site of the coarctation repair, involving the left subclavian artery (Fig. 1). This was treatedwith a complex hybrid thoracic endovascular aneurysm repair [1].

Percutaneous endovascular repair of aneurysm after previous coarctation surgery.
BACKGROUND Formation of aortic aneurysm late after surgical repair of coarctation carries a significant risk of rupture and lethal outcome, and repeat surgery is associated with a 14% in-hospital mortality rate and morbidity from paraplegia, injury to the central nervous system, or from bleeding. The potential of nonsurgical endovascular repair by the use of stent-grafts in lieu of repeat surge...
متن کاملSingle stage repair of a complex pathology: end stage ischaemic cardiomyopathy, ascending aortic aneurysm and thoracic coarctation
The not uncommon combination of ascending aortic pathology with late presenting coarctation is a difficult surgical challenge. The two stage approach is usually adopted. The necessity for cardiac transplantation adds to the complexity: a trans-sternal approach and single stage repair become mandatory.
متن کاملAORTIC TRANSECTION AND ITS SURGICAL REPAIR: A CASE REPORT FROM IRAN
Aortic transection is a rare but life-threatening complication of blunt chest trauma. Treatment options include open repair, medical management and most recently, endovascular repair. We report a 33 years old male with a traumatic aortic transection following a car accident. Aortic injury and its pseudo-aneurysm were confirmed by Chest CT scan and TEE in the descending thoracic aorta just belo...
متن کاملLong-term results of endovascular aortic repair for thoracic pseudoaneurysms after previous surgical coarctation repair.
OBJECTIVES Late aneurysm formation has been reported after every type of surgical coarctation repair in up to 10%, with rupture of such aneurysms being responsible for approximately 7% of all deaths. Secondary surgical repair carries a significant mortality (up to 15%) and morbidity rate (recurrent laryngeal nerve paralysis ∼20%, phrenic nerve injury ∼5%). According to the positive experience w...
متن کاملMycotic aortic aneurysm as a postsurgical complication: report of a case and review of the literature
Mycotic aneurysms are localized and irreversible dilatations of the arteries caused by weakening and damaging the arterial wall by an invasive organism establishing infective arteritis. Mycotic aneurysm of the thoracic aorta is a rare event; however, it can be fatal if not diagnosed early or not treated appropriately. Clinical findings are usually nonspecific; however, c...
